To determine the association between individual substances of abuse and antiretroviral adherence, analyses require a large sample assessed using electronic data monitoring (EDM). In this analysis, EDM data from 1,636 participants in 12 US adherence-focused studies were analyzed to determine the associations between recent use of various substances and adherence during the preceding 4 weeks. In bivariate analyses comparing adherence among patients who had used a specific substance to those who had not, adherence was significantly lower among those who had recently used cocaine, other stimulants or heroin but not among those who had used cannabis or alcohol. In multivariate analyses controlling for sociodemographics, amount of alcohol use and recent use of any alcohol, cocaine, other stimulants and heroin each was significantly negatively associated with adherence. The significant associations of cocaine, other stimulants, heroin, and alcohol use with adherence suggest that these are important substances to target with adherence-focused interventions.

When I was 14 I tried crack cocaine, then I started using e and powder cocaine, on my 15th birthday I smoked crack again, and went on a serious 2 week drug and drinking binge, we took 20+ pills of e a day and did around 5-10 grams of powdered cocaine a day. I was admitted into a detox program on Christmas eve (my birthday is November 21) and state clean until febuary of this year when I started taking e again, then began drinking again and eventually began doin powdered cocaine everyday, I stopped taking e when I overdosed at the beginin of this summer, and stopped drinking when I almost died from sever alcohol poisioning. I soon after quit cocaine. About a month or two before my 16th birthday I began using cocaine on a regular basis again. And would aprox’ do 4-7grams a day. I quit for a few weeks and began using again, but as of today I am 5days sober. This is the longest I’ve had withdrawl symtoms and their much worse, I puke for hours at a time, and since I barely eat it’s mostly stumach bile with blood. I’m wondering if my body will ever go back to normal and if I can do anything to help it heal?

Secreted aspartic proteases (Saps) are extracellular proteolytic enzymes that enhance the virulence of Candida pathogens. These enzymes therefore represent possible targets for therapeutic drug design. Saps are inhibited by nanomolar concentrations of the classical inhibitor of aspartic proteases pepstatin A and also by the inhibitors of the HIV protease, but with the K(i) of micromolar values or higher. To contribute to the discussion regarding whether HIV protease inhibitors can act against opportunistic mycoses by the inhibition of Saps, we determined the structure of Sapp1p from Candida parapsilosis in complex with ritonavir (RTV), a clinically used inhibitor of the HIV protease. The crystal structure refined at resolution 2.4 Å proved binding of RTV into the active site of Sapp1p and provided the structural information necessary to evaluate the stability and specificity of the protein-inhibitor interaction.
